WHR describes where you carry weight. A higher ratio means more abdominal fat relative to hip fat, which is linked to higher cardiovascular risk.

Why distribution matters more than total

Two people can weigh the same, stand the same height, and carry the same total body fat, and face meaningfully different cardiovascular risk. The difference is where the fat is.

Fat stored under the skin on the hips and thighs is comparatively inert. Fat stored inside the abdomen, packed around the liver, pancreas and intestines, is not: it is metabolically active tissue that releases free fatty acids directly into the portal circulation and secretes inflammatory signalling molecules.4 Waist-to-hip ratio is a crude but effective proxy for how much of your fat is the second kind.

The evidence that made this mainstream

INTERHEART enrolled around 27,000 people across 52 countries and found waist-to-hip ratio a substantially better predictor of myocardial infarction than BMI — the association held after adjustment, and across every region studied.2 That is an unusually robust result for an anthropometric measure.

Worth noting: the Emerging Risk Factors Collaboration, pooling 58 studies and over 220,000 people, found that once blood pressure, cholesterol and diabetes status were accounted for, adding waist measures to BMI did not improve cardiovascular risk prediction much further.3 That does not make the measurement useless — it means its value is in flagging people who need those other things checked, rather than in the ratio itself being a separate disease.

Measuring it properly

The ratio is only as good as the two measurements, and both are easy to take badly.

  1. Waist. WHO specifies the midpoint between the lowest rib and the top of the hip bone — not the navel, and not where your trousers sit. Find both landmarks with your fingers first.
  2. Hips. The widest part of the buttocks, tape horizontal all the way round. Check in a mirror that it has not ridden up at the back.
  3. Breathe normally. Measure at the end of a relaxed exhale. Do not hold your breath, suck in, or push out.
  4. Tape snug, not tight. It should sit flat against the skin without compressing it. Measure over bare skin or one thin layer.
  5. Repeat. Take each measurement twice; if they differ by more than a centimetre, take a third and average.

Why the navel is the wrong landmark

Navel position varies between people and moves as abdominal fat changes, which makes it unreliable for tracking your own progress over time. The rib-to-hip midpoint is a skeletal landmark and stays put.

What to do with a high ratio

Visceral fat responds well to the things that are good for you anyway — and, usefully, it tends to go first when you lose weight, which means early progress is disproportionately the fat you most want to lose.

  • A modest overall deficit. Visceral fat is mobilised readily. The Calorie Calculator sets a sustainable target.
  • Regular aerobic exercise. Reduces visceral fat measurably even without much weight change.
  • Sleep and alcohol. Both are strongly associated with abdominal fat accumulation, and both are easier to change than most people expect.
  • Get the associated markers checked. A high ratio is a good reason to have blood pressure, lipids and fasting glucose measured — which, per the Emerging Risk Factors work, is where the actionable information really is.3

Subcutaneous fat
Fat stored directly under the skin. The kind you can pinch. Comparatively benign, and the dominant store on hips and thighs.
Abdominal obesity
Excess fat concentrated around the midsection, defined by waist circumference or waist-to-hip ratio rather than by total weight. A person of normal BMI can have it.
